Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
For your convenience, we accept cash, personal checks, debit cards, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

Dental InsuranceIf you have dental insurance, as a courtesy, we will submit claims for you and estimate benefits. However, you must keep in mind that your insurance policy is a contractual agreement between you or your employer and the insurance company. If for any reason your insurance company rejects your claim or has not made payment within 45 days of billing, you are responsible for the balance. All insurance co-pays are due at the time the service is rendered.
We are a preferred provider for the following insurance companies:
- MetLife Delta Dental
- Anthem Cigna
- United Concordia Aetna
- Dominion
- United Concordia Alliance Network
- Assurant
If you have dental insurance that is not one of the above, we will still be happy to file your dental claim for you. However, you should be aware of your out of network benefits prior to services.
Account Balances
Please understand that payment for your bill is considered part of your treatment. You are responsible for any balances not paid by your dental insurance company. All patients with outstanding balances will receive a monthly statement with payments due on or before the 20th of every month. Overdue balances will incur a finance charge of 1.5% (18%APR) applied each month the balance remains outstanding. All accounts with unpaid balances after 60 days will be turned over to a collection agency unless previous arrangments have been made with our business staff. Should it become necessary to do so, a 34% service charge (collection fees, attorney fees, and any other charges) will be applied to the unpaid balance, which will also become your responsibility.
Canceled/Missed Appointments
Since we offer our patients the chance to reserve appointments at their preferred times, it is important to keep these appointments as originally scheduled. We make every effort to see our patients promptly at their appointment time. It is your responsibility and commitment to treatment if you reserve a scheduled appointment. We require 2 business days' notice to cancel or reschedule appointments and reserve the right to charge a fee for appointments broken within the 2 day time period. We certainly understand that emergencies occur; however, the time reserved for you could have been given to another patient. Please be courteous not to cancel or reschedule at the last minute.
